Transcript Request Forms for Current Sachem North Students:
Current students who require a transcript and/or letters of recommendation to be sent to a college or scholarship program should use the following form to request their transcript from their counselor:
This form must be printed out and returned in person to the School Counseling Office, where one of our secretaries will provide a receipt for your submission. Do NOT email this form to your counselor.

Some helpful information regarding transcript requests:
For timely processing of your request, make sure you have reviewed the following items. Please abide by the above deadline dates for submission of transcript request forms and requests for letters of recommendation. Requests submitted after these deadline dates may not be able to be processed by the deadline.
- Matching your Common App with your Naviance Account: If you are using the Common Application to apply, please make sure you have "matched" your Naviance account to your Common App by following these steps: FERPA/Matching Instructions
- Add Schools to Common App: If using the Common App to apply, please make sure that all colleges you are currently submitting requests for have been added to your Common Application. We are unable to submit documents to schools that have not been added to your Common App.

Counselor Letter of Recommendation: If you require a letter of recommendation from your school counselor, make sure you have completed the Counselor Recommendation Form. These forms can be found on the School Counseling Google classroom.

Teacher Letter(s) of Recommendation: If you require teacher letters of recommendation, make sure you have reached out to your teacher(s) to ask them to write you a letter (either email or in person).
Please alert teachers to your application deadlines and follow up with them to ensure that their letters have been uploaded to Naviance.

SAT/ACT Testing: If you plan to submit SAT or ACT scores to your colleges, these scores must be submitted directly from the testing agency (SAT - www.collegeboard.org / ACT - www.actstudent.org). We cannot send your SAT or ACT scores. Transmission of your scores from the testing agency can take up to a few weeks, so make sure to request your scores early if you plan to submit them. In recent years, many colleges have adopted test optional policies. Make sure you are reviewing these policies on the college's web site when making a decision about whether or not to submit your scores. If you have questions about this, see your school counselor.
