Health Forms for Try Outs

Click on the appropriate link below to access the necessary medical documents for tryouts. These are the ONLY forms that will be accepted by the district for athletic tryouts.


FinalForms Registration

Every athlete must register with Final Forms. Once registration for sports opens, enroll your child in the sport they are trying out for. You will then be prompted to fill out required forms (ie. parent signature, student signature, update form). All of these forms, in addition to your valid physical, must be completed in order for the school nurse to be able to review/clear your child for sports.

Please note: Steps must be completed in the correct order as listed below. If you do the signatures and update form before adding a sport, adding the sport will reset the signatures and update.

  1. Add Sport
  2. Complete All Forms
  3. Once everything else is in green, the nurse will be able to review/clear your child for sports.

FINAL FORMS registration will open 1 month prior to the start of the season.

**Before handing in physicals, medication orders, or doctor’s notes….

Take a photo of each page of your document(s) (not in “Live Mode”- Final Forms does NOT support video/.HEIC files) and upload them to Final Forms for the nurse to review.

***Anyone looking to register for a sport after the start date of the season must email the coach with your intentions of trying out.

Girls Bowling Places Second in County Championship

The Sachem girls varsity bowling team placed second in the Section XI County Championship on Feb. 10 at Bowlero in Sayville. The team was led by seniors Cira Arcangel and Bella Quaranta.

Senior Cira Arcangel won several awards for the tournament – high series of the tournament, high game with a 279, and second place highest average bowled with a 221.83. Cira finished in the top six girls in Suffolk County and will once again, for the third straight year, be a part of the Section XI All Star Team, representing Section XI in Syracuse on March 8.

Long Island Invitational Girls Wrestling Championships

Sachem High School East student Mia Marchitto competed in the Long Island Invitational girls wrestling championships on Sunday, Feb. 4, 2024. She won her match against Huntington's Aylin Davila Chitay. Mia was recently selected as Newsday’s Athlete of the Week. Check out the full article at the link below:

North Wrestling Team Named League II Champs

The Sachem High School North wrestling team won the Suffolk County League II tournament title over number two-ranked Connetquot. This is the first league tournament title for the Arrows since 2013 and only the second title since the split of the high schools. Sachem North was paced by their three league champions, sophomore Jack Cusumano, junior Joe Marotta and senior captain Nate Gsell. In total, the Arrows qualified 13 wrestlers for the Suffolk County Championships being held at Stony Brook University this Saturday, Feb. 10 and Sunday, Feb. 11. All 13 place finishers include:

Board Celebrates Fall Athletics Success

During the Jan. 24 meeting of the Sachem Board of Education, the board and administration celebrated a successful fall athletics season. Several fall teams made it to the playoffs and dozens of student-athletes were awarded with post-season honors. Additionally, every Sachem team earned the designation of a scholar-athlete team for the fall 2023 season. This honor from the New York State Public High School Athletic Association recognizes teams with 75% of their roster having a combined GPA greater than or equal to a 90.

The board first recognized four Sachem coaches who received post-season Coach of the Year honors. Congratulations to East girls varsity volleyball coach Amanda Schmohl, East girls junior varsity volleyball coach Christina Loffredo, North varsity field hockey coach Carly Sharp and North football coach Dave Caputo. Following the coach recognitions, students who received post-season honors including All-County, All-Division, All-League, All-Long Island and All-State were celebrated.

Next, Kristen Fieger and Clodagh Harte were honored for being named Teachers of the Year from the New York State Association for Health, Physical Education, Recreation and Dance Suffolk Zone. Ms. Fieger from Samoset Middle School was named the Middle School Health Education Teacher of the Year and Clodagh Harte from Tamarac Elementary School was designated Elementary School Physical Education Teacher of the Year.

The Suffolk Zone also thanked the district for hosting its annual conference at Sachem High School East this past November. More than 1,100 health and physical educators, as well as athletic directors, came together at High School East for a professional conference filled with 60 presentations taking place across the building. Suffolk Zone President Lori Hewlett presented the district with a plaque as a token of appreciation for its hospitality and advocacy.
