Directions, maps and schedules to athletic events are available at Section XI.

Health Forms for Try Outs

Click on the appropriate link below to access the necessary medical documents for tryouts. These are the ONLY forms that will be accepted by the district for athletic tryouts.

 

FinalForms Registration

Every athlete must register with Final Forms. Once registration for sports opens, enroll your child in the sport they are trying out for. You will then be prompted to fill out required forms (ie. parent signature, student signature, update form). All of these forms, in addition to your valid physical, must be completed in order for the school nurse to be able to review/clear your child for sports.

Please note: Steps must be completed in the correct order as listed below. If you do the signatures and update form before adding a sport, adding the sport will reset the signatures and update.

  1. Add Sport
  2. Complete All Forms
  3. Once everything else is in green, the nurse will be able to review/clear your child for sports.

FINAL FORMS registration will open 1 month prior to the start of the season.

**Before handing in physicals, medication orders, or doctor’s notes….

Take a photo of each page of your document(s) (not in “Live Mode”- Final Forms does NOT support video/.HEIC files) and upload them to Final Forms for the nurse to review.

***Anyone looking to register for a sport after the start date of the season must email the coach with your intentions of trying out.

Sports Start Dates

Building nurses will be available to clear students for sports participation on the following dates this summer:

High Schools: 7:30am-2:00pm only
August 15, 16, 19, 20, 22, 23, 26, 27

Middle Schools: 7:30am-2:00pm only
August 20, 21, 26, 27

Fall Sports Start Date
High School: Football 8/19/24; All other Sports 8/26/24
Middle School: 9/6/24

Winter Sports Start Dates
Early Winter Middle School: 11/4/24
Late Winter Middle School: 1/13/25
High School: 11/18/24

Spring Sports Start Dates
High School: 3/17/25
Middle School: 3/24/25

On June 11, senior student-athletes from Sachem East and Sachem North were recognized by Section XI with The Gold Key award. The Gold Key is one of the most established and recognized awards in Suffolk County athletics. It represents an athlete’s dedication and commitment to athletics throughout their high school career.

In order to be recognized for the Gold Key Award, an athlete must have participated and lettered in eight of nine seasons while enrolled in grades ten through twelve. This requires a commitment by the athlete while maintaining a positive balance between athletics and academics.

Charlie Johnson Track Meet

Sachem middle school student-athletes competing in the annual Charlie Johnson Track and Field Meet. thumbnail259692

Sachem middle school student-athletes competed in the annual Charlie Johnson Track and Field Meet last month, honoring the longtime teacher and coach of the Sachem Central School District. Johnson coached middle and high school cross country, winter track and spring track. He was inducted into the Sachem Athletic Hall of Fame in 2022. His legacy lives on as student-athletes come together each year for this meet in his name.

Long Island Champs

Sachem East varsity softball team thumbnail259205

The Sachem East varsity softball team won the Long Island championship on May 31. The team will now go on to compete for the New York State championship this weekend. We wish the team the best of luck!
