Records for Former Students

Former students requesting an official high school transcript:

• Please call the Guidance Office at 631-716-8200 ext. 5148 at least 24 hours prior to picking up an official transcript. Attached is the Transcript Request form needed to release your transcript. Transcripts can be picked up between the hours of 7:00 a.m. and 3:00 p.m., Monday through Friday.
• Please bring a form of identification with you (must be a picture ID).
• If you prefer to return the form via mail, please complete the form and enclose a copy of your driver's license along with cash or money order made payable to: Sachem High School in the amount of $3.00 per transcript up to $9.00.
