Sachem Middle School Early Winter Sports Registration

Dear Sachem Athletics Families,

Registration for Winter Sports will open in Final Forms 30 days before the start of each season on the following dates:

• Middle School Early Winter: October 9th 
  • Boys Basketball
  • Girls Volleyball
  • Girls Competitive Cheer

🔄 Registration Process

1. Add Sport your child plans to try out for
2. Complete All Required Forms (parent & student signatures, update form, etc.)
3. Submit Valid NYS Physical and any necessary medical documents

a.   2026 NYS Health Exam

b.  Medical Documents 

Important: Complete all steps in this order.
If you complete forms before selecting a sport, your signatures and forms will reset when you add the sport.

✅ Clearance Process

• Once all forms are complete and highlighted GREEN, the building nurse can begin reviewing your child's documents.
• Clearance is not immediate — you will receive an email confirmation from Final Forms once your child is cleared.
