Registration for Winter Sports will begin through Final Forms on the following dates:

Early Winter Middle School Sports- October 4th

V/JV Sports- October 18th

Late Winter Middle School Sports- December 13th

If you have not done so, please register at https://sachem-ny.finalforms.com/ and review the Parent Playbook (English)/(Spanish) for assistance.

Once registration for sports has begun, you must enroll your child in the sport they intend on trying out for before filling out any forms. Once registered you will then be prompted to fill out all required forms (ex. parent signature, student signature, update form). All of these forms, in addition to a valid NYS physical and any other necessary medical documentation, must be completed for the school nurse to review/clear your child for sports.

ALL DOCUMENTS ARE SUBMITTED AND REGISTRATION IS DONE THROUGH FINAL FORMS ONLY.

Please note: Steps must be completed in the correct order as listed below.

* Complete all Forms with the most current student information
* Once all forms are highlighted green, the building nurse will review/clear your student for sports.

***Please understand that when everything is complete (ALL sections are signed by parent/guardian and student) nurses can review and the process will not be completed until a nurse reviews all information.
